Looking at what the system held after the customer's answer separates the five causes immediately.
Nothing was stored
Where the slot is empty after the customer answered, the value was either not extracted or was rejected, which are different problems.
Checking whether a validation error was logged distinguishes them without further investigation.
Something was stored and then lost
Where the value was captured and is absent later, the state was cleared between steps, which is a flow transition problem.
This is the most common cause in deployments with several linked flows.
It was stored and still asked
Where the value is present and the question is asked anyway, two flows are both requesting it and one is not checking whether it already exists.
This is a configuration overlap rather than a technical failure.
Stored locally but not saved
Where the conversation holds the value but a backend write failed, the next step reading from the backend finds nothing and asks again.
Integration errors around this are frequently silent, which is why this cause is often missed.
